Application Forms

All visa applicants are required to submit an application at the time of the interview. Do not send any form directly to the embassy. In addition to the basic application form, certain applicants are required to submit additional supplemental forms. Applicants should refer to the following guidelines to determine which forms to fill out and submit.

  1. ALL APPLICANTS: Fill out the EVAF DS-156 visa application form in English. A Hungarian language guide to filling in the form is provided below.

  2. ALL MALE APPLICANTS BETWEEN THE AGE OF 16 AND 45: In addition to the DS-156, fill out the DS-157 supplemental form.

  3. APPLICANTS OF VISA CATEGORIES F (academic or language study), M (nonacademic or vocational studies), and J (exchange programs, fellowships, au pairs, etc): In addition to the DS-156, fill out the DS-158 supplemental form.

The required application forms can be found below:

Please note the following when completing the application forms:

  • All questions MUST be answered. Incomplete applications will be rejected. If there is no answer to a question, fill in the space with "N/A."

  • Check the application and supplemental forms for accuracy and completeness before printing.

  • The embassy only accepts forms that are filled out electronically. No handwritten forms will be accepted.

  • In item 14: "National Identification Number" of the DS-156, please enter your ID card number (személyi igazolvány).

  • Important: Please make sure the barcode on the last page of the EVAF is legible. If the barcode is smudged or unreadable, please print the form one more time. If the problem still persists, we ask applicants to please refill the form again.
