Lost or Stolen Passports
If you lose your passport, a replacement passport can be issued upon presentation of proof of identity and U.S. citizenship (i.e. birth certificate, driver's license, or previous passport). In addition to the ID, two 5 cm x 5 cm (or 2"x2") photos with a light background will be required.
If the passport has been stolen, first report the theft to the local police and obtain a police report for presentation to the consular officer.
